EXHIBITOR’S CONTRACT

CCOSA/OMLEA Middle Level Conference –

Friday February 29^th  and Saturday March 1^st , 2008

Double Tree Hotel-Downtown Tulsa-616 West 7^th Street-Tulsa, Oklahoma 74127

Terms and Conditions:

1.  The CCOSA/OMLEA will bear the expenses and responsibility of setting up the exhibit space.

2.  The exhibitors will be responsible for setting up and dismantling their individual exhibits.

3.  Each exhibitor is asked to provide a prize to be given away Saturday, March 1^st  , 2008.

4.  Each exhibitor will receive one box lunch on Friday afternoon from OMLEA.

5.  Exhibitors will be responsible for their exhibit during the conference schedule.

6.  Special time will be allotted during the conference schedule for booth visitation.

7.  Remit payment of $200 with contract.  Make checks payable to CCOSA/OMLEA.  No refunds.

8.  Exhibits may be set up Friday morning, February 29^th  at 7:00 a.m. and dismantled at 12:00 p.m. on Saturday, March 1^st .

9.  Please let us know beforehand if you need electricity or extra tables/chairs in your booth space.

10.  Pipes, curtains, 1 table (skirted and draped) and 2 chairs will be provided.

11.  All exhibitors will be listed in the program if information received by January 15, 2008.

If it is necessary for you to ship materials to the hotel, each item must be properly packed and marked with:    

1) your organization’s name and contact        
2)date of function-March 1^st , 2008 and OMLEA                           
3) name of hotel contact (TBA later)                                                                                                    

Boxes, packages, and display material will be accepted no earlier than two (2) days before the scheduled event.  The hotel reserves the right to refuse to accept packages that appear damaged, and assumes no liability for the condition of the content of such packages.

“The exhibitor assumes the entire liability and responsibility for losses, damage and claims arising out of injury or damage to exhibitors’ displays, equipment, and other property brought upon the premises and shall indemnify the Double Tree Hotel, its employees and agents from all losses, damages and claims.”

In accordance with terms and conditions set forth by CCOSA/OMLEA, the organization so stated is scheduling exhibit space offered at the CCOSA/OMLEA Conference.  Payment must be included with this application.
